GROUP HEALTH PLAN (FOUNDATION ONE):

All temporary-assignment personnel who work a minimum of 30 hours per week and have satisfied the 30 day waiting period are eligible for this pre-tax medical indemnity plan. This plan has no pre-existing condition limitations and provides coverage for:

  1. Office Visits
  2. Accidents
  3. Prescription Drugs,
  4. Lab and X-ray
  5. Hospitalization

SUPPLEMENTAL PLANS (AFLAC):

All temporary-assignment personnel who work a minimum of 30 hours per week and have satisfied the 30 day waiting period are eligible for additional products through AFLAC which includes:

  1. Life Insurance
  2. Disability Insurance
  3. Dental Insurance

401(k) PLAN

All temporary-assignment personnel who are at least 18 years of age are eligible to enroll in the 401(k) plan immediately.  Plan highlights include:

  1. Immediate Eligibility
  2. Automatic Payroll Deductions
  3. Pretax Deductions
  4. Contributions can be 1% to 20% of eligible pay
  5. Annual Increase Program (automatically increase contributions)
